Medisave Payment
1. Chronic Disease Management Programme (CDMP)
a) Under this programme, the Ministry of Health (MOH) allows the use of medisave for outpatient treatment for the following conditions:-
  • Diabetes
  • Hypertension (also known as 'High Blood Pressure')
  • Lipid Disorders (also known as 'High Blood Cholesterol')
  • Stroke
  • Asthma (applicable for both adults and children)
  • Chronic Obstructive Pulmonary Disease (COPD)
If you suffer from any one of the above condition(s) be it an existing illness or a newly diagnosed one, you can participate in this programme.

b) How can I join this programme?
Simply complete the Medisave Authorisation Form (MAF)
[Kindly refer to the top right hand corner of this page for the MAF link] 
and submit it to our clinic with the co-payment that is stipulated by MOH.

c) Annual Withdrawal limit (w.e.f 1st January 2012)

There is an annual withdrawal limit of SGD400 per medisave account per calendar year at the GP clinic. Patients can also use their immediate family member(s)' medisave up to a maximum of 10 accounts. e.g. their spouse, child(ren)..

2. Vaccinations
a) MOH have extended the use of medisave for the following vaccinations at the outpatient GP clinics:-

  • Hepatitis B Vaccinations
  • Human Papillomavirus (HPV) Vaccinations against cervical cancer. For HPV vaccinations, Medisave payment can only be used for young female aged 9-26 years as stipulated by MOH.
  • Pneumococcal Vaccines. Medisave payment can only be used for children below the age of 6 years old with effective from 1 January 2012. 
b) How can I use my medisave to pay for the vaccinations?
  • Simply complete the Medisave Authorisation Form (MAF)
    [Kindly refer to the top right hand corner of this page for the MAF link] 
    and submit it to the clinic.
  • The amount deducted will be based on the price of the vaccines.
  • Parents can use their medisave account for their children's vaccines. 
c) Annual Withdrawal Limit (with effective from 1st January 2012)

  • There is an annual withdrawal limit of SGD400 per medisave account per calendar year at the GP clinic.
  • Patients have the option to use their immediate family member(s)' medisave.
